The increasing prevalence of chronic disease in the developed world is alarming. Hypertension now affects up to 20% of adult Australians, diabetes 10% (Ausdiab) and obesity and overweight almost half the Australian adult population. Does chronic disease management comprise the majority of your case load these days? Have you identified this area as a major learning priority for yourself? If so, this may be the course for you.
Course estimate: 24+ hours of online learning, plus a one-day practical skills workshop, personal study time and assessment, for a total of approximately 50+ hours.
This course is the same asThe University of Queensland course GENP7002 Chronic Disease Management in General Practice in the Master of Medicine (General Practice). Satisfactory completion results in a Statement of Satisfactory Completion from The University of Queensland School of Medicine and is recognised for credit toward the Master of Medicine award program*.
The explosion in the prevalence of chronic disease provides numerous challenges to GPs - clinical challenges in terms of the management of multi -system disease, and implementation of clinical pathways and protocols; and organisational challenges in the optimal delivery of services across the practice team in providing patients with optimal care. Finally, it requires good practice skills in fostering patient skills in lifestyle management and chronic disease self-management.
This course will equip students with the ability to develop and utilise advanced knowledge and skills in the implementation of chronic disease management approaches in the primary care setting, including how to effectively utilise a team approach. Students will consider the impact of chronic disease on the individual, their families/carers and the society as a whole and the evidence behind various strategies to enhance chronic disease management. Students will also develop skills in supporting self-management principles for people with chronic disease. The course contains a one-day workshop where students will be able to liaise with other health-care providers regarding effective chronic disease management strategies.
Teaching and Learning Modes
Students in this course will have access to 12 online tutorials. These online tutorials will have didactic and interactive components. Students are expected to spend 1.5-2hrs working through each tutorial plus additional study time. Students will be expected to read and listen to lectures, read additional material outlined in the tutorials, such as articles and texts, and contribute to peer discussions. Students will also actively participate in a one-day interactive group workshop, available in May and November.
Assessment details
Online Learning Log Book to be completed progressively throughout the course and submitted online or at the workshop (pass/fail)
Examination testing theoretical knowledge to be conducted at the beginning of the workshop (40% of assessment)
Competency assessment of clinical skills to be held at one-day workshop (pass/fail)
Assignment to be submitted after workshop (60% of assessment)
The assessment methods will allow the student to demonstrate proficiency in both theoretical and practical aspects of the course as outlined in the learning objectives.
*NOTE: If you intend to seek credit for this course, and you intend to complete more than two courses in the Master of Medicine (General Practice) program through UQ Health Insitu, seek advice from the program manager as some administrative pre-conditions apply.

At the completion of this course, students will be able to:
demonstrate advanced knowledge and skills in the description and implementation of chronic disease management approaches in the primary care setting, including established and emerging models, and their evidence base
demonstrate clinical proficiency in the management of common chronic disease processes in the general practice setting, including diabetes and congestive cardiac failure
demonstrate an understanding of relevant research and new models of care , and application of knowledge to appropriate choice of treatment / therapeutic modality.
demonstrate knowledge of the epidemiology and impact of chronic disease in Australia and internationally and potential future issues facing societies
collect appropriate data in general practice relating to chronic disease management and utilise the principles of information management to improve chronic disease outcomes in your clinical setting
define practice systems that can enhance effectiveness of care of people with chronic disease and how to implement such systems
demonstrate effective utilisation of a team approach to optimise care of people with chronic disease
demonstrate knowledge of the principles of self-management approaches to chronic disease and how to facilitate and support self-management strategies for people with chronic disease
assess and demonstrate knowledge and understanding of the important issues and impact of chronic disease on people at all stages of life, including children, adolescents and the elderly, and their families/carers
describe best-practice chronic disease management approaches in general practice and develop approaches to make successful linkages between best available evidence and clinical practice

Audience
The student must be a qualified medical practitioner with a minimum of 2 years clinical experience in general practice. It is highly desirable that the student be currently working in a primary care setting.
